Our pride in offering customized medications requires extra attention to detail and time; therefore please allow 48 to 72 hours for your compound to be prepared.
Please fill out the form below, if you have any special request please enter them in the “comments” section of the form. By leaving your email we will be able to respond back to your email address confirming we received your request and other important information. Once form is completed click on submit. After you submit your form you should see a “thank you for submitting” page.
If you would like your prescription mailed please indicate that in the form and fill out your mailing address. Shipping cost may vary depending on storage requirements per compound.
